Everyone needs to have a Living Will and a Health Care Proxy/Agent that specifies your wishes on what type of medical care you desire at the end of your life (Living Will) and who you designate to be the person that will help to ensure your wishes are followed (Health Care Proxy). This website was written and designed for NY state residents with only a handful of states where it cannot be used (listed on the site). For instance, it says that in my home state of NC it must be witnessed by 2 people and notarized.
Who should have a copy of these?
- Any doctor that you see on a regular basis
- Designated person to act on your behalf (Health Care Proxy)
- Immediate family members
- Hospital(s) where you’re likely to receive treatment
- To the attorney that prepared your Living Will, Last Will and Testament, so it can be attached.
